The Vermont Department of Health (DOH) is requesting feedback from people with professional and/or lived expertise on the draft Vermont State Health Improvement Plan (SHIP) to make sure that it is clear and meaningful. Visit the DOH webpage to view a short slide deck that provides an overview of the draft SHIP and how it was developed. After reviewing the slide deck, please complete this survey by February 28 to share your thoughts.
What is the Vermont State Health Improvement Plan?
The SHIP is a five-year roadmap for ensuring that all people and communities in Vermont have inclusive, equitable, and sustainable access to opportunities for health and well-being. It reflects the shared efforts of people and organizations across the state. It outlines what the DOH plans to achieve, how they will achieve it, and how they will know if they have achieved it.
What are the SHIP's Goals?
There are four areas of focus in the SHIP:
- Access to Care: Increase access to inclusive, equitable, and affordable health care services.
- Cost of Living: Improve health and quality of life by addressing the impact of the high cost of living.
- Housing: Improve the availability of affordable, accessible, and safe housing.
- Mental Health & Substance Use: Strengthen the capacity of the mental health and substance use services system to support individuals and communities.
The SHIP includes strategies for how the State will achieve its goals and the State's indicators of success.
What Will Happen to My Input?
Workgroups will update the plan based on feedback received. The State Health Improvement Plan will launch in April 2025. Workgroups will continue to meet over the five-year period of the plan to implement and coordinate efforts.
